For sure 3.5. Judgement call on whether the dentine on the 5th tooth (2nd molar) is as wide or wider than enamel. If so, 4.5. If not, 3.5. Given the width of the dentine on the 1st molar and the seemingly wide-enough dentine of the 2nd molar, I'd go with 4.5. For sure not 5.5 or older using the Severinghaus technique.
