Form between photgrapher and subject

Does anyone have a form they use (or know where to get one) that basically is signed by myself and the person(s) being photographed?

All I need (I think) is something pretty basic. I won't do this with the photos, they won't do that with the photos with out the other's written permission.

This is getting very close....... Now I just need it to say that THEY can not use the photos for anything other than personal use without written permission or something along those lines.
