I would have it checked but should be okay. Bolt might just need a new extractor.

Good question. Doesn't look like web separation to me. Looks like it was twisted off with pliers.

I twisted the casing off trying to get it out.

Any diagnosis with just that picture would be pure speculation. It does look like that brass was distorted by punching it out or twisting AFTER it was fired, not as a result of.
