It was not marked correctly, but if I remember correctly our insurance settled with him just to get him to go away. Wasn't our fault...was our problem.

Hopefully no one was injured and things can get cleaned up properly considering. I worked in a refinery setting for 34 years plus and in the best laid plans problems and mistakes occur. And of course then there are the folks you work with that you wonder how they ever got hired. Lol The Monday morning quarter backs are usually in full attendance also.
