We ended up reading up on them on http://www.texasnfatrust.com/fingerp...-fd-258le.html then ordered the cards and ink and did our own. I filed for two form 1's which took about a month and once I knew they were accepted, I filed for two form 4's.
Identogo.com is another option. they have offices all over.
Yeah I've had one form kicked back because of a little hiccup. I'd rather a professional handle it. Although when I took my criminal justice classes I had to do a finger printing section and did fairly well on cooperative subjects. Uncooperative not so much.

Was the form kicked back for fingerprint issues?
There is a dealer forum I follow and the question came up a while back asking if any dealer had ever had a F4 kicked back for fingerprints. None had. The consensus was that they can be pretty bad and still make it through. YMMV Rolling your own should be fine if you know what you are doing.
