Writing a business plan

I am needing to construct a business plan for a business loan. I could google all day, but I prefer real world application. I would like to hear about what some of you have put in your business plans and been successful in securing funding. Needing to know what to put in and what not to put in. Thanks!

Executive summary
Product/Service description- what you are going to provide, how it will differentiate in the market
Market analysis- target market, size, trends, competition
Operational plan- what’s needed to run the business
Marketing plan- Print, TV, Social media, etc., how you’ll reach your target market in a cost efficient way
Pro-Forma Financial statement- revenue, expense, income, NPV
Development Plan- What is needed to launch the product/service

I've done many, and this is pretty spot on. Might add a page on the Team and their experience if that's a favorable attribute.
