Truck going sideways down road - what causes it?

Driving down i35 saw this truck - it really was this bad. I know bent frames cause the truck to look like it going sideways but will a bad lift job do it as well? Just had never seen one so bad.

He needs to make some adjustments to the blocks in the rear.
He didn't make any measurements to make sure the distances were
the same on the right and left side. Her just threw the blocks in and
tightend things down.

I have seen this with stock trucks and most of the time its FORD . Just saying ,,, keep an eye out see how many you see like this and how many are FORD
