Looks like a goiter to me, it will make him really thin and look like he is about to die. Then it will bust and fall off and he will be back to normal.

A goiter is the animal has iodine deficiency which causes thyroid issues in the animal. Now typically you see them on the neck where the thyroid is located, but if the deer has a wound it will pop up there. I had one on my lease last year and it was on his belly.
