The two knobs are probably antler with velvet still on them. It's not exactly common, but not real uncommon either. Don't know what they're called, but I've seen pics of quite a few, and seen a very few in person.

I believe the black round patch by his left eye is some sort of a slight infection as if maybe he got hooked on a fence or a thorn of some kind. Not so sure about the nub at the base of the antler. Perhaps a late growing antler. If you can keep this young lad in your pictures for a couple of months or so, you may be able to tell more as time goes on.
